Commitment Agreement And Liability Release Agreement

By submitting this information, I hereinafter referred to as registrant am committing to raise a minimum of $250 for For His Glory Outreach and participate in the Frederick Running Festival May 5th and 6th, 2012. Frederick Church of the Brethren and For His Glory Outreach (the "Sponsors") are providing assistance in arranging this Event.

The Sponsor and the registrant agree that this event poses risks, including injury and sickness as well as similar and dissimilar risks (the "Risks").

For and consideration of the Sponsors assisting the participant in the Event, the registrant, for himself/herself and representatives, heirs, guardians and next of kin (the "Releasers"), hereby releases and covenants not to sue the Sponsors and their affiliates, members, directors, officers and agents (the "Releasees"), for and from all claims now and in the future, including, but not limited to, all liability to the Releasers on account of injury to the registrant or death to the registrant or injury to the property of the registrant, whether caused by the negligence of Releasees of otherwise, while the registrant is participating in the Event. The registrant is fully aware of the Risks and voluntary assumes the Risks of loss, damage or injury that may be sustained by the registrant while participating in the Event.

The registrant also agrees that he/she bears the sole responsibility for any and all medical expenses which he/she incurs while participating in the Event, whether for injury or illness, and whether required as a result of the registrant's participation in the Event or not. The registrant acknowledges Releasees are under no obligation to, and do not, provide medical insurance for the registrant. The registrant will not hold any health care member responsible in any circumstances for any medical service received.

The registrant acknowledges Releasees are under no obligation to, and do not, cover travel expenses for the registrant.

The registrant further warrants that he or she has fully read and understands this Commitment Agreement and Liability Release Agreement and voluntarily signs the same, and that no oral representations, statements or inducements apart from the foregoing written agreement have been made to the registrant.

By submitting the requested information, the registrant accepts the terms and conditions of this agreement.
